The following forms must be completed to participate in programs at the USOTC in Colorado Springs and are also required for athletes, staff and personal coaches traveling with USAW teams to international events:

1) Participant Bio

2) Medical History

3) HIPAA

4) Waiver of Liability & Code of Conduct for USOTC Program

5) Code of Conduct  for International Team Travel

If your organization or athlete wants to compete in a non-USA Weightlifting sanctioned event outside the U.S., you must complete the following form and submit it to the National Office at least 30 days before participation in the event.
